CV Mekar Jaya is a company that operates in the field of goods sales and repair services with a vision and mission to provide the best and excellent service. This company has problems in the repair system which manually collects data which is a problem, so that if a disaster occurs consumer goods data will be lost due to lack of good storage, therefore we need a goods damage reporting system that can cover the needs of the company and consumers in order to supports performance to be even better. In the research method, using the Waterfall Method for system development consists of analyzing system requirements, designing the system using UML, implementing the system based on the design created, testing using black boxes and finally maintenance. A web-based application built with `. The results of this research can help companies use an information system to report damage to goods that will be used. In the end, the website-based goods damage reporting system that was built will be used to support the smooth reporting of goods damage to CV. Mekar Jaya
