Medical records are closely related to the Minimum Service Standards for Puskesmas. which is evidence of the process of medical service to patients. The form of the Medical Record is in the form of a manual, which is written completely and clearly and in electronic form according to the provisions. Medical records consist of patient data records carried out in health services. These records are very important for services for patients because complete data can provide information in determining decisions about treatment, treatment, medical action and others. The purpose of this study is to determine the productivity of outpatient services and to facilitate decision making (planning, organizing, implementing, monitoring, evaluating, and controlling) by clinical and administrative service providers in health care facilities. The benefits of designing this application are to provide information to facilitate management in services to patients and to carry out the treatment service process with the current system used so that it is hoped that it will be easier to find data on patients who have received outpatient treatment at the Puskesmas. The design of this system will use Unified Modeling Language(UML), and design using android studio.
