This study aims to apply the concept of DMAIC and FMEA to control the quality of crude palm oil (CPO) with specifications of free fatty acid content, moisture content and impurities content. DMAIC is a tool or systematic method used for process improvement and product development based on statistical methods. FMEA is used to identify and analyze potential failures for improvement. The results of the study show that there are still data outside the specification limits, so improvements need to be made. In the calculation of process capacity, there is a Cpk value of less than 1 which indicates a process produces a product outside of specifications. The sigma value of the measurement results is 3.01. The root causes of declining quality such as labor, materials, machines and methods are then analyzed using FMEA. From the results of FMEA, the highest RPN value is 252 and the cause of failure is poor quality of raw materials which is the main priority for the type of failure to be repaired immediately. Proposed product quality improvement with 5W+1H by correcting all factors that cause quality decline by conducting training on the performance of the workforce, upgrading machines to function optimally, and even checking all levels of raw materials because products are produced according to company specifications and inspection standards, products must be increased regularly to maintain stability over process variations.
