The work is very dangerous, the emergence of work accidents at work will harm the workforce and an unsafe and unhealthy work environment also has an impact on the disruption of labor performance. Therefore, when carrying out work is required to implement an Occupational Safety and Health (K3) system. This study aims to determine the identification of Occupational Safety and Health (OHS) risks to worker performance on the Sigli-Banda Aceh Toll Road project, Aceh Province. The method in this study used a questionnaire totaling 35 respondents distributed to business entities, contractors, independent quality control consultants, and supervision consultants. This questionnaire is to determine the identification of Occupational Safety and Health (OHS) risks, namely the likelihood of work accidents, the severity of work accidents and the value of categories in the form of tables using a scale value of 1: 5. The results of the question from the likelihood of work accidents value 35%, from the severity of work accidents value 49%, and from the value of high and low risk categories value 58%-42% can be concluded on the Sigli-Banda Aceh Toll Road project in the application of Personal Protective Equipment (PPE), Safety Protective Equipment (APK), and Occupational Safety and Health (OHS) standards in accordance with procedures so that work risks that occur can be minimized.
