The Teratai Medical Center Clinic located in Medan is engaged in the service sector, namely the medical field. The clinic was built with the aim of assisting government programs in the health sector. Currently, the handling of patient medical records is still manual, namely the doctor records in handwriting every examination result of the patient's medical record data in the status folder until the records become one folder. When a doctor wants to see a patient's previous progress, he has to look up the patient's history one by one in that one folder and convey it to the patient, which takes a long time. Another problem encountered is the vulnerability to loss of medical record data reports caused by negligence by officials in storing or destroying them when needed. The loss of patient medical records makes it difficult for doctors to determine medical action. With an information system built, it can help clinics in the process of health services (patient administration, pharmacies and medical records) and reports.This information system is built on a Web basis using the Model View Controller and the SDLC system development method.
